EBM Medical is committed to quality improvement and providing a
high standard of quality care to patients.

If you have a problem you would like to discuss, please feel free to talk to our
administration staff. We take your concerns, suggestions, and complaints seriously.
You may also send a written complaint to the Practice Manager who
will complete an appropriate investigation and inform you of the outcome.

From time to time we invite patients to complete questionnaires on their view of the service provided. These questionnaires are completely confidential and help us to improve our services. Regardless of whether you are happy or unhappy about a service provided, we are interested to know. Feel free to discuss this with the Doctor/Practice Manager or submit your comments in the ‘Suggestion Box’ in the waiting room.

We strive to resolve all issues and complaints within the practice in the first instance, however should you wish to take the matter up externally you may contact:

Every effort is made to accommodate your preferred time and Doctor. Longer consultations are available, so please ask our receptionist if you require extra time.

We are happy for patients to contact us to see if there have been any delays. In the event of major delays our reception staff will attempt to contact you.

EBM Family Medical Practice is a mixed billing practice. Dr Senthil Govindasamy charges a gap fee of $20 for new patients and $10 for existing patients for his consults. All other doctors in the practice Bulk-Bill patients holding a valid Medicare card. For patients not holding a valid Medicare card a minimum $37.05 fee is payable at the time of the standard consultation.

Pre-employment medicals, Commercial Drivers Licence renewals and some procedures/services are not covered by Medicare and will incur a practice fee. Our reception staff can advise you of the practice fees charged for these services.

We ask for at least one hours notice prior to appointment cancellations or a fee may be charged for non-attendance.

If you or a family member requires an interpreter, we can organise this for you.
Please let us know when you book your appointment.

We post letters to remind you when certain tests or checks are due
(e.g. pap smears, diabetes checks). If you do not wish to receive such letters, please advise
your doctor or the reception staff. We follow the State Territory & National Recall Systems.

In line with the National Privacy Principles – your medical records are confidential documents.
It is policy of this practice to maintain security of personal health information at all times and
to ensure that this information is only available to authorized members of staff.

All patient consultations and any information regarding appointments are also kept strictly
confidential. It cannot be divulged to anyone other than the patient without written authorization.
This limitation extends to family, friends and employers.

There are systems in place to ensure that any abnormal results are identified and acted upon immediately. Our staff will be in contact with you if your results require follow up.

It is best that patients book an appointment to follow up their results with the Doctor for advice and management. Test result processing time can vary from two to seven days and your patience during this time is greatly appreciated.

Smoking is prohibited within EBM Family Medical Practice’s premises. This includes all outside areas within the fencing perimeter, all offices, waiting, treatment and consulting rooms
